Affinity Disability Services offer a range of in home supports for people with a disability. Our support empowers participants and builds capacity for people with a disability.Our support services include:
Support Coordination
Support Coordinators will help the participant to better understand the NDIS, pricing and budgets, bring the participants plan to life and enhance the participants understanding of relevant and available services. They will support the participant to link in with services, strengthen new and existing networks, ensure the participant receives quality supports and services and also prepare the participant during the plan review process.
Psychosocial REcovery Coaching
A Psychosocial Recovery Coach is specifically designed for participants who have a primary diagnosis of psychosocial disability. Recovery coaches offer a more hands-on approach, using their specific knowledge of mental health services, to ensure the participant receive the best support to reach their goals. They will coach participants to have greater participation in managing their lives, collaborate with the participant and their support network to design, plan and implement a recovery plan.
Daily Activities
Affinity Disability Service workers will support the participant to learn and engage with daily tasks such as cooking, cleaning, and household chores. This support will educate participants and empower them to take control of their daily lives. We also support our participants to complete daily living tasks such as cleaning, grocery shopping, and attending appointments.
Household Tasks
We will support our clients to complete everyday tasks that they are unable to do. This includes meal preparation and delivery, house or yard maintenance, and cleaning.
Transport & Accomodation
We will support our clients with travel and transport to things like appointments, events and gatherings. We will also provide support to manage their home and living environment, accommodation and tenancy needs.
Social & Community Access
We will support our clients to engage with their community as well as family, friends and other support networks through involvement in community events, and social activities. Additionally we help our clients build capacity to engage and join-in.
